This patch contains fixes for the majority of remaining reported issues!

Changelog:
- Fixed Door Ram not stunning nearby NPCs
- Fixed players teleporting to the corner of the map when changing equipment while throwing
- Fixed throwing delay and accompanying visual glitch
- Fixed small, invisible holes in some walls that could be seen through
- Fixed NPCs getting visually stuck in stunned state when stunned while stunned
- Fixed NPCs occasionally being semi-visible when they should not be visible
